• Estimated Weekly Pay: $1,687 – $1,874\n
• Schedule: Monday – Friday, 8:00 AM – 4:30 PM\n
• Shifts per Week: 5\n
• Shift Duration: 8-hour days\n
• Hours per Week: 40\n
• Weekend Requirement: One weekend shift per month\n
• On-Site Position\n
• Floating: Required to float to sister healthcare campuses as needed\n

\n\nRegistered Nurse Qualifications\n

• Minimum 2 years of recent Case Management RN experience required\n
• Active Registered Nurse (RN) License (must be verified on Nursys and included in your profile)\n
• BLS certification required\n
• Care Management certification (ACM or CCM) preferred\n
• Degree accreditation must be included in your profile\n
• Ability to float to other healthcare campuses as needed\n
• Permanent address must be at least 50 miles from the facility (travel nursing requirement)\n
• All demographic information (address, email, full DOB, full SSN, RN license number) must be provided at time of application\n
• Hard document deadline is the Monday prior to start date\n

\n\nCase Management RN Responsibilities\n

• Deliver comprehensive case management and clinical care coordination services to patients, ensuring continuity of care and optimal outcomes\n
• Collaborate with interdisciplinary healthcare teams to develop, implement, and evaluate individualized patient care plans\n
• Coordinate patient transitions across care settings and manage discharge planning\n
• Advocate for patients and families, connecting them with appropriate healthcare resources and support\n
• Maintain accurate documentation and comply with all regulatory and hospital standards\n
• Float to sister campuses as required, functioning as a core staff member and supporting a positive healthcare team culture\n
