Situated in the city of Delray Beach, the Center provides a broad spectrum of inpatient and outpatient care to clients and their families in surrounding communities.
Job Summary:
The Intake and Evaluation screener is a member of the multi-disciplinary team who provides quality care for the clients in the Acute Care units within the inpatient and outpatient setting. The Intake and Evaluation screener provides Crisis Screening Assessments when needed. He or She works within the Intake unit and Outpatient setting in accordance with Standards of State and Federal Regulations. Complete interviews with clients coming into CSU, completing all paperwork required Provide patient care according to established Policies and Procedures under supervision of Acute Care Director and Director of Nursing. Daily reporting required.
Job Responsibilities:
- Assess all individuals requesting emergency services
- Respond to crisis situations, conduct risk assessments, and complete safety planning with individuals in crisis.
- Conduct mental health screenings for patients with mental health seeking inpatient services.
- Conduct mental health screenings for patients with mental health seeking outpatient services.
- Identify appropriate level of care (LOCUS)
- Assesses patient's psychosocial, economic, and cultural needs and collaborates with care teams, patients, families, and physicians to develop/ alter plans of care in order to restore and/ or maintain patient mental health status so that the plan of care reflects multidisciplinary evaluation and intervention. The Intake Screener will collaborate with the other treatment team members to develop the plan of care.
- Completing Risk Assessments including but not limited to PHQ-9, GAD-7, C-SSRS, DAST-10, Stanley Brown
- Completing client inventory.
- Have the knowledge and ability to interpret Baker
- Ability to write assessments in a clear and concise format
- Comfortable working in a crisis setting.
- Ability to manage crisis calls and situations
- Attend I & E Team Meetings at SCMHC
Job Requirements:
- Previous experience in mental health is preferred
- Must be willing to learn
- Strong organizational and communication skills, must have proper grammar
- Experience with Credible a plus
- Ability to thrive in a fast-paced environment
- Team Player
- Must possess a valid driver’s license and current automobile liability insurance.
- Must be able to utilize equipment such as computers, faxes, copiers, etc.
- Be able to function in an inbound call center environment.
- Bilingual a plus (Spanish, Haitian Creole)
Job Qualifications:
- A Bachelor’s Degree or higher in the behavioral health field (i.e. social work, psychology, sociology, counseling, etc.) or a Bachelor's degree with 30 hours of helping-profession coursework.
- A Master’s Degree or higher in the behavioral health field (i.e. social work, psychology, sociology, counseling, etc.), preferred.
Written & Verbal Communication
- Ability to document accurately.
- Ability to communicate will with staff.
Functional, Specialized & Technical
- Ability to organize
- Ability to perform tasks that are expected of nursing staff
Working Conditions
- OSHA Classification – Class III
Minimum Education & Experience
- Bachelors in the Mental Health Field $20
- Masters in the Mental Health Field $25
