Location
This position is based in Crestview, FL, providing occupational therapy services to Covenant Home Health Care patients through our partnership with Concierge Home Care.
Your Role as a Home Health Occupational Therapist
As a Home Health Occupational Therapist, you will deliver high-quality, individualized therapy services in patients’ homes—helping them regain independence, improve safety, and enhance overall quality of life.
Key Responsibilities Include:
- Traveling to patients’ homes to provide occupational therapy services
- Conducting comprehensive evaluations of patients’ functional abilities and home environments
- Developing and implementing personalized, goal-driven treatment plans
- Educating patients and caregivers to promote independence and successful outcomes
- Collaborating with patients, caregivers, Covenant leadership, and the Concierge clinical team
- Supervising and delegating care to Certified Occupational Therapy Assistants (COTAs), as applicable
- Completing timely, accurate documentation using streamlined tools designed to maximize patient-facing time
Qualifications
- Unencumbered Florida Occupational Therapy license (required)
- Valid driver’s license, reliable transportation, and active auto insurance (required)
- CPR certification (preferred)
- Ability to supervise and delegate to Certified Occupational Therapy Assistants (preferred)
- Home health experience (preferred, but not required)
