Position Summary:
We are seeking a detail-oriented and highly organized Clinical Intake Coordinator to join our team in Boca Raton, FL. This role is responsible for managing multiple referral sources and portals, processing clinical documentation, and ensuring all admissions meet Medicare regulations and agency standards. The ideal candidate thrives in a fast-paced environment and understands the importance of accuracy, compliance, and timely communication.

Key Responsibilities:


  • Receive, review, and process referrals from multiple sources and electronic referral portals
  • Verify patient eligibility, payer requirements, and physician orders
  • Ensure all intake documentation meets Medicare guidelines and home health compliance standards
  • Coordinate with clinical staff, physicians offices, hospitals, and referral partners to obtain complete and accurate documentation
  • Communicate admission status, missing information, and next steps to internal teams and referral sources
  • Maintain accurate and timely documentation within the EMR system
  • Support timely start of care and assist with census management
  • Adhere to HIPAA and agency confidentiality policies at all times

Qualifications:


  • Minimum of 35 years of experience in home health intake, admissions, or a related healthcare coordination role
  • Strong working knowledge of Medicare home health requirements and documentation standards
  • Experience managing multiple referral portals and high-volume intakes preferred
  • Familiarity with EMR systems and electronic referral platforms
  • Excellent organizational, communication, and problem-solving skills
  • Ability to prioritize tasks and meet deadlines in a fast-paced environment

Preferred Qualifications:


  • Experience with Medicare-certified home health agencies
  • Clinical background (LPN) a plus!
  • Knowledge of insurance verification and authorization processes
