We are seeking a Team Coordinator to provide vital administrative support to our care teams and leadership. This role is central to keeping records accurate, meetings organized, and communication flowing smoothly across the hospice team. The Team Coordinator ensures that behind-the-scenes operations run seamlessly so that patients and families receive timely, high-quality care.

Overview

The Team Coordinator is responsible for providing administrative and operational support to the hospice interdisciplinary group (IDG) and leadership team. The position ensures that patient and program records are maintained in accordance with federal, state, and organizational requirements, prepares and organizes IDG meetings, tracks physician orders, updates the electronic medical record (EMR), and assists with compliance activities.

Key Responsibilities

  • Maintain communication with agency leadership to ensure quality of care and effective business performance. Professionally represent the organization to the public, while promoting optimal customer service at all times.
  • Prepare and organize documentation and reports required for IDG meetings, ensuring compliance with regulatory requirements.
  • Track, verify, and follow up on physician orders to ensure timely completion and proper documentation; fax orders to the Medical Director for signature, to facilities as needed, and verify aide order frequencies.
  • Maintain and update patient records in the EMR according to federal, state, and organizational standards; be proficient in the EMR and serve as a resource for team members.
  • Upload, review, and file documentation in the EMR as directed by leadership.
  • Update the clinical record as needed and directed.
  • Assist with intake processes when the Care Navigator is unavailable, ensuring accurate and timely data entry.
  • Generate and distribute operational and compliance-related reports as requested by leadership.
  • Coordinate with the Medical Director and facility contacts to manage documentation flow and confirm receipt of signed orders.
  • Develop relationships with team members to serve as a resource for administrative and EMR-related needs.
  • Maintain confidentiality of all patient, family, and personnel information in accordance with HIPAA regulations.
  • Support leadership with administrative tasks, special projects, compliance initiatives, and report preparation.
  • Participate in staff meetings, department meetings, team meetings, briefings, in-services, and other related activities as needed.
  • Answer and route incoming calls to the appropriate staff member and ensure timely follow-up.
  • Assist in preparing documentation for audits, surveys, and quality initiatives.
  • Perform other duties as assigned.

Qualifications

  • High school diploma or equivalent required; associate degree or higher preferred.
  • Minimum two (2) years of relevant medical office or healthcare administrative experience preferred; hospice or home health experience strongly preferred.
  • Proficiency in Microsoft Word, Outlook, Excel, and EMR systems.
  • Strong organizational skills with the ability to manage multiple priorities and deadlines.
  • Excellent verbal and written communication skills.
  • Ability to work independently with minimal supervision.

The starting compensation for this position is shown below. Arc Hospice is committed to transparency and equity in all compensation practices. The rate displayed reflects the starting hourly or salaried pay for the role and does not represent a range.
