Job Description

Direct Care & Program Staff provide daily supervision, care, and support to residents in a group home environment. This role is responsible for assisting individuals with activities of daily living (ADLs), implementing individual service plans, promoting safety and independence, and supporting residents’ emotional, behavioral, and social well-being in accordance with agency policies and regulatory standards.

Essential Duties and Responsibilities

Resident Care & Support

  • Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility as needed
  • Support residents with meal preparation, feeding assistance, and household routines
  • Encourage independence, self-advocacy, and skill development
  • Provide emotional support and maintain a respectful, person-centered approach

Program Implementation

  • Implement Individual Service Plans (ISP), Behavior Support Plans, and treatment goals
  • Support residents in community activities, appointments, school, work, and recreation
  • Model appropriate behavior and social skills
  • Maintain structured routines that promote stability and consistency

Health & Safety

  • Administer medications or medication reminders in accordance with training and regulations
  • Monitor residents for changes in physical, emotional, or behavioral health
  • Follow crisis intervention and de-escalation protocols when necessary
  • Maintain a safe, clean, and organized living environment
  • Follow infection control, emergency, and safety procedures

Documentation & Compliance

  • Complete daily logs, incident reports, medication records, and progress notes accurately and on time
  • Report incidents, concerns, or suspected abuse/neglect immediately per policy and law
  • Maintain confidentiality and comply with HIPAA and agency standards

Team Collaboration

  • Communicate effectively with supervisors, clinicians, case managers, and families/guardians
  • Participate in team meetings, training sessions, and performance evaluations
  • Accept feedback and demonstrate willingness to learn and improve
