A-Line Staffing is seeking a Case Management Coordinator to join our Case Management team. The Coordinator will conduct telephonic and face-to-face assessments to evaluate member needs and coordinate appropriate services for Medicaid Long Term Care/Comprehensive Program enrollees. This position supports both supportive and medically complex members while focusing on wellness, integration of care, and long-term positive outcomes.
This role requires a professional who is self-motivated, organized, tech-savvy, and able to work independently while managing a dynamic caseload.
Pay: 34.13/hr
Location: Candidates must reside in Miami-Dade County
Duties & Responsibilities:
- Conduct comprehensive evaluations using case management tools and data review
- Coordinate and implement individualized care plan activities and monitor progress
- Conduct multidisciplinary reviews to support optimal member outcomes
- Identify, track, and escalate quality of care issues
- Use motivational interviewing to promote member engagement and healthy behavior changes
- Provide coaching, resources, and education to empower members in healthcare decision-making
- Coordinate services such as prior authorizations, provider communication, condition management resources, community supports, and medication review
- Collaborate with PCPs, skilled providers, and community partners
- Conduct face-to-face visits in members’ homes, ALFs, and SNFs
- Maintain compliance with regulatory guidelines and accreditation standards
- Utilize Microsoft Office tools including Excel and Teams
- Effectively multitask and adapt to a fast-paced environment
Required Skills & Qualifications:
- Bilingual (English & Spanish), fluent in speaking and writing
- Strong verbal and written communication skills
- Ability to multitask, prioritize, and adapt to change
- Strong organizational skills and attention to detail
- Professionalism, strong work ethic, and positive attitude
- Ability to work independently in a remote/field environment
- Tech-savvy; able to use Microsoft Teams, Excel, and online platforms
Experience:
- Case Management experience required
- Long Term Care experience strongly preferred
- Microsoft Office (Excel, Teams) required
- Must be comfortable with extensive field travel (up to 75%)
Education Requirements:
- Bachelor’s degree in Social Work or related field required
- No nursing candidates will be considered
- High school diploma or GED required
- No prior CVS/Aetna experience required
Additional Details:
- Candidates must reside in Miami-Dade County
- Mileage reimbursement available
- Fully remote role with 50–75% local travel
- Training conducted remotely via Microsoft Teams
- Not a Peak Season position
