ABOUT THE ORGANIZATION:

Established in 1970, Boley Centers, Inc. is a private, non-profit organization serving individuals with mental disabilities, individuals and families who are homeless, Veterans and youth in Pinellas County. Boley Centers, Inc., a nationally recognized and accredited behavioral health care agency, has an opening for a full-time Licensed Practical Nurse.

JOB SUMMARY:

The LPN is responsible for managing medication forms at each Residential Treatment Facility (RTF) group home and coordinating services between Evara and Boley RTF staff. The LPN also provides a range of case management, education, treatment, rehabilitation and support services to assigned consumers. Due to the nature of the program being a 24/7 program, the LPN may be required to work outside business hours, weekends, holidays and on-call. Works in conjunction with the RTF RN.

EDUCATION AND EXPERIENCE:

  • Current LPN licensure in Florida.
  • At least 1 year of psychiatric nursing experience in an in-patient or out-patient mental health setting, working with Severe and Persistent Mental Illness (SPMI) clients or substance abuse treatment.
  • Knowledge of psychotropic medications, coordinating home health care, severe and chronic mental disorders.

RESPONSIBILITIES:

  • Initiate, update and revise medication observation records in electronic health record (EHR) and on MOR paper forms.
  • Provide input to treatment team for client treatment planning.
  • Provide case management services when required in conjunction with Care Coordinators and Medical Office staff.
  • Monitor medication regimen in conjunction with Evara and manage pharmaceuticals and medical inventory at each RTF.
  • Act as liaison with community agencies and families to coordinate treatment and services.
  • Educate consumers regarding medication compliance and potential side effects.
  • Evaluate and follows up on any medical concerns.
  • Operate own or agency vehicle to travel between locations.
  • Attend and participate in staffing and treatment planning meetings.
  • Provide direct clinical services to consumers in individual and group settings.
  • Maintain productivity level and write/maintain required records in time frame defined by management.
  • Utilize knowledge of Medicaid Community Mental Health Services Handbook to document and bill for services provided.
  • Demonstrate safety awareness.

SPECIAL REQUIREMENTS:

Valid Florida driver's license, own transportation vehicle and vehicle insurance required.
