Physician – Skilled Nursing Facilities
Location: Port Charlotte, FL
Schedule: Monday – Friday | Day Shift
Employment Type: Full-Time
Compensation: $225,000 – $250,000 annually (based on experience)
Position Overview
We are seeking a compassionate and clinically strong Physician (Family Medicine or Internal Medicine) to provide medical care across multiple Skilled Nursing Facilities (SNFs) in Port Charlotte. This is an excellent opportunity for a physician who enjoys longitudinal patient care, team-based practice, and a predictable weekday schedule.
You will work alongside experienced mid-level providers who support charting and lower-acuity visits, allowing you to focus on higher-complexity clinical decision-making and patient outcomes.
Travel between facilities within a 50-mile radius is required and fully reimbursed.
Key Responsibilities

Provide comprehensive medical care to skilled nursing facility patients

Perform evaluations, follow-ups, and care plan oversight

Manage chronic and acute conditions with strong clinical judgment

Collaborate with interdisciplinary care teams to optimize patient outcomes

Supervise and work in conjunction with mid-level providers

Document care accurately using the PCC EMR system

Maintain high standards of patient communication and professionalism

Requirements

MD or DO
-
Board Eligible or Board Certified in Family Medicine or Internal Medicine

Active or eligible Florida medical license

Strong interpersonal skills and patient-centered approach

Experience in SNF, post-acute, or long-term care preferred (not required)

ECFMG and ACN providers open for discussion
