Pinnacle Home Care, Florida’s largest Medicare-certified home health provider, has been delivering high-quality, patient-centered care for over two decades, and we’re looking for a Jr. Care Coordinator to join our award-winning team.
Key Responsibilities
- Conduct introductory welcome calls to patients, providing a positive first experience and setting expectations for services.
- Verify physician signatures on orders and follow up as necessary to ensure timely completion.
- Maintain ongoing communication with patients regarding their status and care coordination needs.
- Collaborate with physicians to verify surgical protocols and required documentation prior to patient admission.
- Provide updates to physicians, providers, hospitals, and other healthcare partners regarding patient status.
- Fax and email physician orders to obtain necessary signatures.
- Travel to hospitals, physician offices, or facilities as needed to secure signed orders (location-dependent).
- In certain cases, travel to meet patients onsite at hospitals or facilities. (location-dependent).
- Escalate challenges or barriers to the branch location or leadership team in a timely manner.
- Work collaboratively across departments to ensure smooth patient transitions and resolve issues efficiently.
- Attend meetings to discuss patient status as well as challenges that need to be addressed.
- Manage and maintain an up-to-date patient tracking list, including monitoring discharge dates, orders, and location changes.
- Ensure accuracy and organization of patient records and documentation across multiple computer systems.
- Support compliance with internal processes and healthcare regulations through accurate data management and communication.
Qualifications
- Must have a GED or high school diploma. A bachelor’s degree in a related field is preferred. A nursing background is a plus.
- Strong organizational skills with excellent attention to detail.
- Customer-service–oriented mindset with the ability to communicate professionally and compassionately.
- Comfortable working in multiple computer systems and digital platforms.
- Ability to multitask and adapt to changing patient or provider needs.
- Willingness to travel based on location needs (when applicable).
- Ability to collaborate effectively with internal teams, physicians, and healthcare partners.
- Exhibit organizational skills, professional appearance, behavior, and a service attitude toward the community and others.
- Must possess excellent prioritization and time management skills.
