Clinical Supervisor – Home Care
Location: Fort Myers, FL
Position Type: Full-Time | Field-Based with Some Office Responsibilities
Schedule: Monday through Friday, 8:00 AM – 5:00 PM
We are a national leader in home care and home infusion services, dedicated to delivering high-quality, patient-centered care. We are seeking an experienced and motivated Clinical Supervisor – Home Care to join our team in Fort Myers, Florida. This is an excellent opportunity for a Registered Nurse looking to step into a leadership role while remaining clinically engaged in patient care.
Position Overview
The Clinical Supervisor – Home Care is responsible for the clinical oversight and management of an assigned patient caseload, supervision of field-based caregivers, and delivery of direct patient care as needed. This role ensures compliance with all applicable federal, state, and local regulations, accreditation standards, and company policies. The Clinical Supervisor collaborates closely with physicians, interdisciplinary care teams, and the National Infusion Manager to support high-quality patient outcomes. This position is primarily field-based with some office responsibilities.

- Up to $85,000, based on experience

Key Responsibilities
- Take, review, and convert home care referrals
- Conduct patient assessments, develop individualized plans of care, and perform occasional home visits
- Coordinate services and discharge planning
- Complete admissions, pre-admissions, and recertification visits
- Manage physician orders and ensure timely implementation
- Collaborate with physicians and interdisciplinary care teams to ensure quality, patient-centered care
- Provide direct patient care as needed, including:
- Infusions (all intravascular lines and multiple drug classifications), Injections, and Wound care
- Participate in on-call rotation as required
Supervision & Clinical Leadership
- Supervise and support field-based caregivers
- Provide clinical leadership and serve as clinical backup when needed
- Ensure compliance with agency policies and all local, state, and federal regulations
- Maintain accurate and timely clinical documentation
- Participate in Quality Assurance and Performance Improvement (QAPI) activities
Requirements
- Current, unencumbered Registered Nurse (RN) license in the state of Florida
- Minimum of two (2) years of current RN experience
- BSN preferred
- At least one (1) year of home health or related experience
- Experience with infusions using all intravascular lines and multiple drug classifications
- Prior supervisory experience preferred
- Strong clinical judgment, leadership, and problem-solving skills
