We are seeking a compassionate and detail-oriented Patient Care Advocate to join our team. This individual will serve as a critical liaison between healthcare providers and our internal teams, ensuring smooth communication, and supporting the delivery of Durable Medical Equipment (DME) fulfillment process. This full-time office role reports directly to the manager of operations and offers a competitive salary and comprehensive benefits package.
Essential Duties and Responsibilities
Serves as the primary point of contact for health providers
Establish and maintain strong, long-term relationships with providers to promote our DME services
Stay up to date with the latest advancements in diabetes management and treatment to provide informed assistance to patients and the healthcare team.
Facilitate communication between healthcare providers and internal teams, ensuring prompt resolution of any service issues, delays, or product inquiries.
Educate our providers on DME product offerings, usage guidelines, and compliance requirements.
Work with providers to secure the necessary prescriptions, clinical notes, and other supporting documentation required to fulfill Continuous Glucose Monitor orders.
Review medical documentation, including progress notes, lab reports, and prescriptions to ensure they meet clinical qualifications for per insurance requirements.
Adhere to all privacy regulations and always maintain confidentiality of patient information.
Other duties as assigned.
Requirements:
Prior experience in a healthcare or DME environment, particularly with obtaining medical documentation, is a plus.
Strong attention to detail.
Comfortable spending the majority of the day assisting patients and providers via phone.
Strong interpersonal and communication skills to effectively interact with patients, healthcare professionals, and other team members.
Compassionate and empathetic nature, with a genuine desire to help patients manage their diabetes effectively.
Excellent organizational skills and attention to detail.
Ability to work efficiently in a fast-paced environment.
Ability to work in an office full time with cross functional teams.
High school diploma or equivalent.
