The Center for Family and Child Enrichment (CFCE) is dedicated to helping children and their families by providing the right services and solutions based on individual needs. CFCE is constantly evolving to better support our community. As a Nurse Home Visitor, you will function under the collaborative practice agreement with a physician, provides professional medical services in the community health center setting.

Some of the Functions Include:
- Promotes personal health, maternal role, environmental health, support from family and friends, life course development and health and human services through assessment, education, promotion of behavioral change, and referral of families for other health and human services.
- Develop therapeutic relationships that promote adaptive behavior change such as decreased substance use, development of healthy relationships and good nutrition.
- Monitor child health, development and growth.
- Teach and model consistent and nurturing parenting.
- Provides health education and treatment of patients within the nurse specialists’ scope of practice using s systematic approach to history and physical appraisal of the patient.
- Demonstrates respect for individuals and diversity; is nonjudgmental, employs active listening skills, is respectful of the client’s heart’s desire, tolerant, understanding, compassionate, encouraging and hopeful.
- Develops and implements patient comprehensive management plans, records progress notes and assists in provision of continuity of care.
- Instructs and counsel patients regarding compliance with prescribed regimens.
- Establishes a collaborative relationship with other medical providers and specialists; determines need for consultation and assists in medical care treatment provided at the direction of other specialists.
- Documents and maintains patient records of services provided according to program standards.
- Participates in education training as assigned.
- Meets metrics set by Nurse Family Partnership and CFCE.
- Other duties as assigned.
Minimum Education
- Graduated from an accredited nurse practitioner program.
- Master’s degree in nursing with advanced skills in physical assessment.
- ANA Certification prior to or within one year of employment.
- Registered nurse with nursing experience in community, maternal or child health or mental/behavioral health.
- A valid Florida Driver’s License.
Other Skills Needed
- Knowledge and skills for the provision of direct patient care in the practice of primary health care.
- Knowledge of the state and federal laws pertaining to patient care and to community health centers.
- Knowledge of the structure and function of community health centers.
- Skill in communicating effectively with patients and their families.
- Skill in establishing and maintaining effective working relationships with other employees, patients and general public.
- Ability to handle confidential information appropriately
- Ability to set appropriate limits and boundaries with clients
- Strong verbal and written communication skills
