At Haven Home Health, we are dedicated to providing exceptional care and support to patients in the comfort of their homes. We are seeking an experienced and highly organized Home Health Intake Coordinator to join our compassionate and fast-paced team.
Position Summary:
The Home Health Intake Coordinator plays a vital role in the referral and admission process. This position is responsible for managing all incoming referrals, verifying insurance eligibility, and coordinating with clinical and administrative teams to ensure smooth and timely admissions. The ideal candidate will have strong attention to detail, excellent communication skills, and a thorough understanding of Medicare home health regulations.
Key Responsibilities:
- Receive and process incoming referrals via phone, fax, and electronic systems
- Verify patient insurance eligibility and authorization requirements
- Accurately input referral and intake information into EMR (Kinnser/WellSky preferred)
- Communicate with referral sources, physicians, and case managers as needed
- Collaborate with clinical teams to schedule start of care visits in a timely manner
- Ensure compliance with Medicare guidelines and all applicable regulations
- Maintain detailed intake logs and follow up on pending documentation
- Provide excellent customer service to referral partners and internal teams
Requirements:
- MUST HAVE , 12 years of skilled Medicare home health intake experience required
- Strong knowledge of Medicare and insurance requirements for home health admissions
- Proficient in EMR systems (Kinnser/WellSky preferred) and Microsoft Office
- Excellent organizational and multitasking skills
- Strong verbal and written communication skills
- Ability to work independently and collaboratively in a team environment
- High school diploma or equivalent; associate degree or higher preferred
