About Us
We are a new hospice organization launching in Miami, dedicated to providing compassionate, patient-centered care with a modern approach. We are building a small founding team of experienced hospice nurses who want flexibility, autonomy, and the opportunity to shape a different kind of hospice.
Role Summary
The Hospice Community Liaison is responsible for developing and managing strong referral relationships with physicians, hospitals, case managers, skilled nursing facilities, and other healthcare professionals. This role directly drives census growth by educating referral sources, ensuring rapid response to referrals, and representing Meridian Hospice as the trusted partner for end-of-life care.
Key Responsibilities
- Build and maintain referral relationships with physicians, discharge planners, social workers, and other healthcare professionals.
- Present Meridian Hospice’s services to hospitals, clinics, and community partners.
- Respond promptly to referral opportunities; coordinate with clinical intake for same-day/next-day admissions.
- Educate providers and community members about hospice eligibility, benefits, and Meridian’s differentiators.
- Track and report referral activity, conversion rates, and market insights.
- Partner with leadership to implement market strategies and achieve growth goals.
Required Qualifications
- Minimum 2 years of experience as a hospice liaison (or equivalent hospice sales/marketing role).
- Established referral relationships with physicians, hospitals, and SNFs in the Miami market.
- Deep knowledge of Medicare hospice benefits and referral/admissions processes.
- Proven track record of meeting and exceeding census growth targets.
- Excellent communication, presentation, and relationship-building skills.
- Valid driver’s license and reliable transportation.
