Job Description

Position Title: Caregiver (Home Health & Home Care Services)

Reports To: Care Concierge Program Manager

Location: Florida, US (On-site)



Job Summary


The Caregiver will provide compassionate, high-quality, and personalized support to patients enrolled in the Care Concierge Program. This role combines traditional home health and home care responsibilities with a concierge approach, ensuring patients and their caregivers receive comprehensive assistance with daily living, health monitoring, care coordination, and social services navigation. The Caregiver acts as a trusted partner to help patients live safely, independently, and with dignity in their homes.


Key Responsibilities:


1. Personal Care & Daily Living Support

· Assist with activities of daily living (ADL's), including bathing, dressing, grooming, toileting, mobility, and feeding.

· Support instrumental activities of daily living (IADL's), such as meal preparation, light housekeeping, laundry, grocery shopping, and medication reminders.

· Ensure a safe and clean environment to prevent falls and accidents.

2. Health Monitoring & Support (Home Health)

· Take and record vital signs (blood pressure, pulse, weight, blood sugar if applicable).

· Monitor changes in the patient’s physical or mental condition and promptly report concerns to the supervising nurse or care manager.

· Support patients with prescribed exercise routines, mobility aids, and rehabilitation activities as instructed by healthcare providers.

· Assist with basic wound care, catheter care, or other delegated medical tasks (as permitted by training and regulations).

3. Care Coordination & Concierge Services

· Serve as the first point of contact between the patient, family, and care team.

· Coordinate appointments, transportation, and follow-up with healthcare providers.

· Help patients and families access community resources (social services, home-delivered meals, financial/insurance support, etc.).

· Maintain up-to-date care notes in the program’s care management system.

4. Companionship & Emotional Support

· Provide companionship, social engagement, and meaningful activities to reduce isolation.

· Offer emotional support to patients and caregivers, fostering trust and dignity.

· Encourage lifestyle modifications (diet, exercise, adherence to care plan) aligned with the patient’s goals.

5. Program-Specific Duties

· Participate in care team meetings and contribute insights on patient progress.

· Educate patients and families on available Care Concierge Program services.

· Support care managers in implementing personalized care plans.

· Uphold program standards of excellence, patient satisfaction, and cultural sensitivity.


Qualifications/Education & Certification

· High school diploma or equivalent (required).

· Certified Nursing Assistant (CNA), Home Health Aide (HHA), or Personal Care Aide (PCA) certification preferred.

· CPR and First Aid certification required.

Experience

· Minimum 1–2 years of experience in care-giving, home health, or related field preferred.

· Experience working with seniors, patients with chronic conditions (diabetes, hypertension, dementia, etc.), or post-acute patients is a plus.


Skills & Competencies

· Compassionate, patient, and service-oriented with excellent interpersonal skills.

· Ability to follow care plans and clinical instructions accurately.

· Strong observation, reporting, and documentation skills.

· Basic proficiency with smartphones, tablets, medical monitoring devices and EHR systems for recording patient notes.

· Ability to manage time effectively and work independently.


Working Conditions

· Patient homes, community settings, and healthcare facilities as needed.

· Flexible scheduling, including evenings, weekends, and holidays, depending on patient needs.

· Physical demands include lifting, transferring, and assisting patients with mobility.

· Adherence to care plans and documentation accuracy.

· Responsiveness to changes in patient condition and timely reporting.

· Contribution to reducing hospital read missions and improving patient outcomes.
