Job DescriptionSwallowing Diagnostics is a leading provider of healthcare services and support, specializing in comprehensive diagnostic services. We're currently seeking a detail-oriented Medical Records Abstractor with strong knowledge of speech-language pathology and primary care medicine to join our dynamic team.

As a Medical Records Abstractor, you will be responsible for reviewing and extracting relevant clinical data from patient health records, with a focus on speech therapy and physician documentation. This role is ideal for someone with a background in health information management and a working knowledge of speech-language therapy terminology, treatment protocols and primary care medicine; with an understanding of how they relate to each other.

Key Responsibilities
Review provided health records and utilize relevant data related to speech therapy evaluations and physician evaluations to build a complete preliminary medical chart for each patient, accurately document HPI and current complaints.

Ensure accurate and timely entry of clinical information into data systems

Collaborate with clinical teams to clarify documentation

Identify trends and gaps in documentation to support compliance and quality improvement

Maintain HIPAA compliance and adhere to all privacy regulations
Desired Qualifications:
Prior experience as a medical records abstractor or in health information management; Strong understanding of speech-language pathology terminology and medical documentation standards
Excellent written and verbal communication skills

Average 6-12 charts to be completed per day; background in speech therapy or related field is highly desirable
