THIS ROLE IS HYBRID....YOU MUST RESIDE IN TAMPA, FL TO BE CONSIDERED FOR THIS ROLE

• Monday through Friday
• 8:00 AM - 5:00 PM EST
• Must be willing to travel to Hillsborough, Highlands, Polk, Hardee, and Manatee Counties for member visits

Job Summary

The Case Manager utilizes a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual’s benefit plan and/or health needs through communication and available resources to promote optimal, cost-effective outcomes.

Job Requirements

• An active and unrestricted RN license for the state of Florida
• An Associate in Nursing is required, a Bachelor's degree is preferred
• 2 years of clinical experience
• 2 years of Case Management experience
• Managed Care experience
• Discharge coordination and transition of care experience
• Home Health experience
• Proficiency in Microsoft Office
• Must be willing to conduct member visits
• Must be bilingual speaking fluently in English and Spanish
