Job Title: LMHC / LCSW Case ManagerLocation: Per diem, every other weekend, shift differential, and on-call bonus
Compensation:$27 per hour (Per-DIEM)

Key Responsibilities:
- Provide individual and family counseling to support patients' mental health and discharge planning.
- Complete intake assessments, psychosocial evaluations, and individualized treatment plans in accordance with hospital policy.
- Demonstrate expertise in the Baker Act and other relevant mental health laws and procedures.
- Act as a resource for the interdisciplinary treatment team on psychosocial factors affecting patient care.
- Monitor compliance with treatment plans and medication in elderly populations, identifying adverse reactions or issues.
- Assess and implement therapeutic interventions tailored to elderly patients’ needs.
- Collaborate with physicians to develop comprehensive psycho-social treatment and discharge plans.
- Initiate and process involuntary examination and Baker Act procedures as required.
- Assist in petitioning for involuntary admissions when necessary.
- Lead and facilitate group and/or individual therapy sessions.
- Provide counseling and discharge support to patients and families for post-hospital care needs.
- Participate in weekly treatment plan reviews and document all services accurately.
- Monitors and ascertains the therapeutic modalities that are conducive to meeting the needs of the patients.
Qualifications:
- Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW) — Required
- Knowledge of Florida mental health laws, including the Baker Act.
- Experience working with geriatric/elderly patients is highly desirable.
- Strong assessment, documentation, and communication skills.
- Ability to collaborate effectively within a multidisciplinary team.
