Job Summary: We are seeking an experienced and knowledgeable Advanced Clinical Pharmacist to join our team. The successful candidate will be responsible for providing advanced clinical pharmacy services to patients and collaborating with other healthcare professionals to ensure the safe and effective use of medications. Key Responsibilities: - Conduct comprehensive medication reviews and provide recommendations to optimize drug therapy - Collaborate with physicians and other healthcare professionals to develop and implement individualized treatment plans - Monitor patients' medication therapy and provide follow-up care as needed - Educate patients and caregivers on medication use, side effects, and potential interactions - Participate in medication reconciliation and medication therapy management programs - Stay up-to-date on the latest developments in pharmacy practice and incorporate evidence-based medicine into patient care - Assist in the development and implementation of pharmacy policies and procedures - Train and mentor pharmacy staff, including technicians and interns - Maintain accurate and complete patient records and documentation - Adhere to all state and federal regulations related to pharmacy practice Qualifications: - Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy - Current pharmacy license in the state of Florida - Board certification in a relevant area of practice (e.g. ambulatory care, geriatrics, pharmacotherapy) preferred - Minimum of 3 years of experience as a clinical pharmacist - Strong knowledge of pharmacotherapy and drug interactions - Excellent communication and interpersonal skills - Ability to work independently and as part of a multidisciplinary team - Proficiency in electronic health record systems and pharmacy software - Commitment to providing high-quality patient care Working at Progeny HR Solutions LLC: At Progeny HR Solutions LLC, we value our employees and strive to create a positive and supportive work environment.
