About the Role

Our client, a well-established and growing healthcare agency, is seeking a Patient Liaison to support ongoing outreach and coordination efforts with patients, providers, and referral sources. This role plays a key part in ensuring smooth communication between patients, physicians, and facilities while maintaining exceptional service standards.

Key Responsibilities

  • Make outbound calls to patients and prospective patients regarding available home health services.
  • Contact doctor’s offices, nursing homes, and hospitals to gather necessary information for new and existing referrals.
  • Provide accurate information to patients about their eligibility and available care options.
  • Verify patient insurance details and coverage through online web portals.
  • Update team spreadsheets and reports to track communication outcomes and referral progress.
  • Utilize multiple software systems to identify and prioritize patients needing outreach.
  • Collaborate with internal team members to ensure timely and accurate follow-up.
  • Perform other related tasks that contribute to patient engagement and provider relations.

Skills and Qualifications

  • Strong interpersonal and relationship-building skills.
  • Excellent written and verbal communication abilities.
  • Proficient computer skills, including Microsoft Office (Word, Outlook, Excel).
  • Highly organized and detail-oriented.
  • Self-motivated, reliable, and proactive.
  • Problem-solving mindset and ability to work independently under moderate supervision.
