About the Role
Our client, a well-established and growing healthcare agency, is seeking a Patient Liaison to support ongoing outreach and coordination efforts with patients, providers, and referral sources. This role plays a key part in ensuring smooth communication between patients, physicians, and facilities while maintaining exceptional service standards.
Key Responsibilities
- Make outbound calls to patients and prospective patients regarding available home health services.
- Contact doctor’s offices, nursing homes, and hospitals to gather necessary information for new and existing referrals.
- Provide accurate information to patients about their eligibility and available care options.
- Verify patient insurance details and coverage through online web portals.
- Update team spreadsheets and reports to track communication outcomes and referral progress.
- Utilize multiple software systems to identify and prioritize patients needing outreach.
- Collaborate with internal team members to ensure timely and accurate follow-up.
- Perform other related tasks that contribute to patient engagement and provider relations.
Skills and Qualifications
- Strong interpersonal and relationship-building skills.
- Excellent written and verbal communication abilities.
- Proficient computer skills, including Microsoft Office (Word, Outlook, Excel).
- Highly organized and detail-oriented.
