Duration: 13 weeks
Shift: Day 5x8-Hour (08:00 – 17:00)

Job responsibilities and all required and preferred skills and requirements:

  Provide in-home physical therapy services to geriatric patients across an assigned territory.

• \t

  Perform comprehensive evaluations, establish plans of care, and deliver skilled PT interventions.

  \t

\t

• \t

  Complete accurate and timely documentation, including OASIS assessments.

  \t

\t

• \t

  Utilize HomeCare HomeBase (HCHB) EMR for all clinical documentation.

  \t

\t

• \t

  Manage an average caseload of 5–7 patients per day.

  \t

\t

• \t

  Collaborate with interdisciplinary care teams to ensure quality patient outcomes.

  \t

\t

• \t

  Travel within a defined territory (up to a 60-mile radius).

  \t

\t

• \t

  Maintain professional standards, licensure, and compliance requirements.

  \t

Required Qualifications:

• \t

  Active Physical Therapist license in hand.

  \t

\t

• \t

  Minimum 1 year of recent home health experience.

  \t

\t

• \t

  Proven experience with OASIS documentation.

  \t

\t

• \t

  HomeCare HomeBase (HCHB) EMR experience required.

  \t

\t

• \t

  BLS certification.

  \t

\t

• \t

  Previous charge experience required.

  \t

\t

• \t

  Ability to work independently in a home health setting.

  \t

\t

• \t

  Reliable transportation with proof of vehicle insurance.

  \t

Preferred/Additional Requirements:

  Experience treating geriatric patient populations.

  \t

  First-time travelers accepted.

  \t

\t

• \t

  Comfort using provided iPad for documentation.

  \t

  Willingness to adhere to dress code (scrubs, no jeans).
