Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Dress Code: Blue scrubs, closed-toe shoes (scrubs provided)Position Overview
We are seeking an experienced Case Management Nurse to provide comprehensive care coordination and patient advocacy. This role involves conducting assessments, developing care plans in collaboration with providers, and ensuring smooth transitions throughout the patient care journey.Key Responsibilities
- Perform comprehensive patient assessments and identify care needs.
- Develop individualized care plans and collaborate with providers to ensure alignment with clinical goals.
- Coordinate services and resources to support patient health and recovery.
- Advocate for patients and facilitate discharge planning for seamless transitions.
- Monitor and evaluate patient progress, adjusting care plans as needed.
Required Qualifications
- Bachelor’s degree in Nursing (BSN) preferred; LPN considered with relevant experience.
- Minimum of 3 years of case management experience.
- Strong knowledge of care coordination and regulatory compliance.
Preferred Skills
- Knowledge of community resources and support programs.
- Critical thinking and problem-solving abilities.
- Positive, motivational approach to patient engagement.
Certifications
- Active RN license in Florida.
- ACLS and BLS certifications required.
