Job Summary:


We are seeking a detail-oriented and compassionate Intake Specialist/Care Manager to serve as the first point of contact for individuals, typically older adults, seeking subsidized services. This dual-role position involves conducting initial assessments, gathering essential client information, connecting individuals to appropriate services or programs, and providing ongoing care management to support clients’ well-being and independence.


The ideal candidate will possess strong interpersonal and organizational skills, a foundational knowledge of human services systems, and a passion for assisting individuals in navigating complex service landscapes. An Associate of Arts (AA) in Human Services is required, along with a genuine commitment to supporting individuals in need.


Key Responsibilities:


Intake Responsibilities:

•Serve as the first contact for incoming clients via phone, email, or in-person visits.

•Conduct intake interviews to assess client needs, eligibility, and urgency of services.

•Collect and accurately document demographic and case information into the agency’s database.

•Explain available programs and services clearly and compassionately.

•Make appropriate internal referrals based on the client’s needs.

•Support clients in completing required forms and documentation.

•Monitor follow-ups for incomplete intakes or additional documentation.


Care Management Responsibilities:

•Develop individualized care plans based on comprehensive assessments of clients’ needs, strengths, and goals.

•Provide ongoing care management, including regular check-ins and reassessments to ensure services remain aligned with client needs.

•Coordinate services with internal programs and external community resources, such as health, mental health, housing, and home care services.

•Serve as an advocate for clients to ensure they receive appropriate, timely, and high-quality services.

•Conduct home visits to assess living environments and ensure client safety and well-being.

•Maintain accurate and up-to-date case notes, documentation, and care plans in accordance with agency and funder guidelines.

•Participate in team meetings to review cases and collaborate on care planning.


Requirements:

Qualifications:


•BSW, LPN or related field required.

•Previous experience in social service or case management role preferred.

•Excellent interpersonal and communication skills, both written and verbal.

•Strong attention to detail and data accuracy.

•Knowledge of social services systems, aging issues, and available community resources.

•Ability to manage multiple priorities and meet deadlines.

•Proficiency in Microsoft Office and client management software.

•Familiarity with Jewish culture a plus.

•Valid driver’s license and ability to conduct home visits.
