Psychiatric Mental Health Nurse Practitioner (PMHNP) – Full Time (Monday-Friday)
Location: Skilled Nursing Facilities (Perry, Live Oak and Tallahassee)

About the Role
PHA is seeking a dedicated PMHNP to join our growing team. You will provide psychiatric care across multiple skilled nursing facilities, collaborating with staff and ensuring the highest quality of patient-centered care. This position offers autonomy, professional growth, and the support of an experienced physician-led group.

Primary Responsibilities

  • Conduct psychiatric evaluations, record patient histories, and maintain accurate documentation.

  • Develop and implement individualized treatment plans in collaboration with supervising physicians and facility staff.

  • Order and interpret diagnostic tests as needed.

  • Prescribe and manage psychiatric medications.

  • Educate patients on health promotion, disease prevention, and psychiatric conditions.

  • Refer patients to specialists when appropriate.

  • Participate in case reviews, consultations, and ongoing care coordination.

  • Adhere to regulatory requirements and reporting standards.

  • Additional duties as assigned.

Qualifications

  • Active State of Florida APRN license in good standing.

  • National Board Certification as PMHNP.

  • DEA license required.

  • Post-acute care experience required.

  • 1 year of psychiatric experience preferred (training available for the right candidate).

  • Proficiency with electronic medical records (EMR).

  • Excellent communication, problem-solving, and teamwork skills.

  • Bilingual (English/Spanish) a plus but not required.

Reporting Structure
Reports to Supervisory Psychiatric Physician / Executive Director

Physical Requirements

  • Effective communication with patients and staff.

  • Ability to sit, stand, or use a computer for extended periods.

  • Manual dexterity for frequent hand and wrist motions.
