Evolve Health is now expanding into other medical service lines to provide our patients with a full range of specialized care. One of the service lines we are developing is Physical Therapy for all types of patients and diagnoses. We are looking for a full-time physical therapist to provide our patients with compassionate high quality of care. Schedule would be Monday through Friday 8 am to 5 pm. We are looking for someone to be flexible with their schedule to provide certain days 10 am to 7 pm to offer patients with after work appointments.
Key Responsibilities:
- Evaluate patients’ mobility, strength, balance and functional impairments.
- Create and implement personalized treatment plans.
- Administer mobility and gait training to help patients with ambulation aids.
- Conduct and document fall risk assessments.
- Document evaluations, treatment plans, and progress into the EMR in a timely manner.
- Provide patients with educational resources and home exercises.
- Collaborate with health care providers, staff, and administration for a successful physical therapy
- program.
- Supervise and train physical therapy assistants.
- Assist management in administrative and clerical function as needed.
- Willingness to participate in ongoing training and program development.
- Ability to maintain confidentiality and comply with HIPAA regulations.

Requirements:
- Active Florida Physical Therapy License.
- Bachelors, Masters, or Doctor of Physical Therapy from an accredited physical therapy program.
- CPR/BLS certification required.
- Dry Needling certification preferred.
