Job Description
We are seeking an OB RN Case Manager to coordinate care for maternal and child health populations. This role ensures safe, evidence-based interventions across the continuum of care and involves intake assessments, individualized care planning, discharge coordination, and ongoing monitoring of clinical outcomes. The position requires strong documentation skills, proficiency with EHR systems, and the ability to manage a high-volume caseload while maintaining quality standards. This role supports compliance with Medicaid contracts and promotes improved maternal and neonatal outcomes.
Responsibilities
<ul>
<li>Coordinate care for maternal and child health populations.</li>
<li>Conduct intake assessments and create individualized care plans.</li>
<li>Oversee discharge coordination and monitor clinical outcomes.</li>
<li>Maintain strong documentation and manage a high-volume caseload.</li>
<li>Ensure compliance with Medicaid contracts and promote improved health outcomes.</li>
</ul>
Essential Skills
<ul>
<li>Active RN license in Florida.</li>
<li>3–5 years of clinical experience in OB, high-risk antepartum, or NICU.</li>
<li>Strong organizational and documentation skills.</li>
<li>Proficiency in care coordination and discharge planning.</li>
<li>Ability to work independently and meet productivity metrics.</li>
</ul>
Additional Skills &amp; Qualifications
<ul>
<li>Bachelor’s Degree in Nursing (Master’s preferred).</li>
<li>Strong communication and interpersonal skills.</li>
<li>Ability to work collaboratively in a multidisciplinary team.</li>
<li>Proficiency with Microsoft Office and EHR platforms.</li>
<li>Flexibility to adapt to changing priorities and hybrid schedules.</li>
<li>Experience with InterQual and motivational interviewing.</li>
<li>Knowledge of Medicaid/Medicare regulations and maternal health quality measures (HEDIS).</li>
<li>Experience in managed care or health plan settings.</li>
<li>Cultural competence and patient engagement skills.</li>
<li>Familiarity with community resources (Healthy Start, WIC, SNAP).</li>

This is a Contract to Hire position based out of Jacksonville, FL.
Pay and Benefits
The pay range for this position is $38.00 - $43.00/hr.
