Job Responsibilities: Access to care:

BETWEEN HIC VISITS:

<li>Assist HIC Provider in communicating with hospitalist/SNF team when patient is referred to ED, admitted to the hospital/SNF, and discharge from ED/Hospital, SNF.</li>

<li>Participate with HIC team in regularly schedules huddles/case conferences to review all/some patients at least&nbsp;once a week.</li>

 Job Qualifications:

 Five (5) years experience in mental health and/or crisis counseling 
 Experience working with adolescents with mental health and social emotional concerns

 Experience with School Based Mental Health Services preferred

Key Responsibilities

<li>Document and update treatment plans and clinical records in adherence to program, regulatory, and confidentiality standards, maintaining accuracy and security.</li>

<li>Lead and facilitate individual and group sessions, including daily process groups and multidisciplinary treatment reviews, promoting open self-disclosure and collaboration on patient needs and goals.</li>
<li>Coordinate resources and develop post-discharge plans to support patients with legal, financial, and vocational needs, ensuring comprehensive recovery planning.</li>

Required Qualifications:
