The Patient Navigator, RN Monday-Friday 8:00 AM-4:00 PM is responsible for working in coordination with care management to assist inpatient and ED patients requiring transport from Lower Keys Medical Center to other hospitals and will support the patient and family members in understanding the care and transport plans. The patient navigator will help navigate and coordinate needed resources such as transportation and temporary housing, supporting the patients return to the Key West community, and assisting when possible with ongoing medical support plans once back in the keys. Requirements
