The Concierge Care Coordination Nurse at the clinic complements the practitioner-patient relationship through support of the established plan of care, using cost-effective, recommended practice guidelines. The goal is to address any acute needs as well as to prevent or delay severe stages of disease progression and enhance the member’s quality of life. In doing so, this position helps to reduce complications and morbidities in an effort to improve health and reduce the costs of the member’s healthcare services.

Position Overview
We are seeking reliable and dedicated Home Health staff ( CNA, HHA,) to join our growing team. The ideal candidate will provide skilled care and support to patients across various settings and must have the flexibility to work both weekdays and weekends as required by patient needs.
Responsibilities
<ul>
<li>Certified Nursing Assistant (CNA) / Home Health Aide (HHA)</li>
<li>Responsibilities:</li>
<li>Assist patients with activities of daily living (ex bathing, grooming, mobility).</li>
<li>Provide companionship and emotional support.</li>
<li>Check vital signs and report changes to supervising nurse.</li>
<li>Maintain safe, clean environment for patients.</li>
<li>Follow care plan under RN supervision.</li>
<li>Must be available weekdays and weekends.</li>
<li>Requirements:</li>
<li>Active CNA license or HHA certificate in Florida.</li>
<li>CPR certification and valid drivers license.</li>
<li>Prior home health or facility care experience preferred ( 6 months)</li>
<li>Participate in weekend and holiday rotation schedule as needed ( mandatory)</li>
</ul>
<ul>
<li>Negative TB test or chest X-ray.</li>
<li>Valid drivers license and reliable transportation.</li>
<li>Clear background check and eligibility to work in the U.S.</li>
<li>Must carry Liability insurance</li>
<li>Ability to work flexible hours including weekdays, weekends, and some holidays.</li>
<li>Strong communication, organizational, and problem-solving skills.</li>
<li>Required to lift up to 50 pounds</li>
</ul>
Preferred Qualifications
<ul>
<li>Previous experience in home health or long-term care ( 6 months)</li>
<li>No prior experience will train on agency protocols</li>
<li>Familiarity with EMR systems/ new hired will be trained</li>
<li>Bilingual (English/Spanish or English/Creole) a plus.</li>

Job Description
Summary: Provides indirect care in the department setting. Monitors cardiac rhythms via direct screen or telemetry monitors and reports arrhythmias to Registered Nurse/and Charge nurse. Meets communication needs of the department. Reports to the Registered Nurse in charge.
Position Qualification/Licenses &amp; Certifications: Attendance and successful completion of: a) Basic EKG course or Telemetry Technician course preferred; advanced EKG preferred. b) 2 weeks telemetry observation with experienced personnel.
<ul>
<li>One (1) year telemetry technician experience in similar job capacity preferred.</li>
<li>Basic computer knowledge.</li>
<li>Requires a through knowledge of basic cardiac monitoring functions and rhythms identification.</li>
<li>Must be able to work rapidly under stressful situations with interruptions.</li>
<li>BLS Certification from the American Red Cross.</li>
<li>Ability to effectively communicate in writing and orally with all levels of the organization.</li>
</ul>
