<li>Perform curative, rehabilitative, or preventive care tasks as outlined in patient care plans.</li>
<li>Utilize clinical expertise and nursing education to make informed, independent decisions that support patient care and promote positive outcomes.</li>
<li>Complete visit reports promptly and accurately within the electronic medical record system to support high-quality care and regulatory compliance, including all required documentation.</li>
<li>Confirm daily visit schedules to ensure seamless coordination with other staff.</li>
<li>Demonstrate awareness of the agency’s Quality Assurance and Performance Improvement (QAPI) program and participate as required.</li>
<li>Understand and adhere to the agency’s process for managing and addressing customer complaints.</li>
<li>Maintain acute awareness of patient rights, ensuring these rights are upheld in accordance with the agency’s service agreements and admission policies.</li>
<li>Understand the agency’s policies regarding appropriate patient discharge and reasons for discharge. </li>

Qualifications
<ul>
<li>Must have a current LPN or LVN license (state-specific).</li>
<li>Graduate of an accredited nursing program.</li>
<li>Valid BLS certification.</li>
<li>Minimum one year of nursing experience (home health or medical/surgical preferred).</li>
<li>Excellent communication, organizational, and time management skills.</li>
<li>Must have a valid driver’s license and access to a reliable and insured vehicle.</li>
<li>Ability to visit homes that may not be wheelchair accessible.</li>
<li>Sufficient hearing, eyesight, and dexterity to perform physical assessments and demonstrate care techniques.</li>
<li>Engage in physical activities such as walking, standing, stooping, and occasional lifting or turning of patients.</li>
