 A EHE RN Medical Care Manager enhances the reputation of AHF by utilizing strong core nursing principles and meeting our patients where they are, identify strengths and opportunities for growth, and assist them with realistic goal setting. With your clinical skills, you will bring the essence of the Ryan White HIV/AIDS Ending the HIV Epidemic (EHE) Initiative - the safety net for uninsured HIV/AIDS patients - to our most vulnerable population. By assessing client needs and developing a coordinated POC, strengthening the HIV Care Continuum by expanding linkage and treatment services using multidisciplinary Intensive Care Teams (ICTs), your work will directly contribute to AHF’s success in meeting the deliverables of our EHE contract. Beyond the scope of the contract, your work contributes to measures established by the HIV/AIDS Bureau (HAB) Measures of the Health Resources and Services Administration (HRSA). You will assure the coordination, follow-up, and completion of medical treatments and/or care plan interventions so that these services ensure timely and coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessments of the client’s and other key family members’ needs and personal support systems. You will verify the client’s eligibility for the ACA (Affordable Care Act) program and enroll those who qualify to ensure broader access to healthcare services.
